Cause of Death

If the cause of death is quite clear the doctor will give you a Medical Certificate and a Formal Notice that the Medical Certificate has been signed.

This is usually the case when someone has been ill for some time and therefore their death has been expected. The doctor who has attended the deceased during their final illness will be able to certify the cause of death and provide the Medical Certificate and Formal Notice.
